Please fill out the information below to log your volunteer hours. Please be specific on your different activities but keep it simple. Read the example below. For each date box, there is a pop up box next to it that you can click on and a mini calendar will show up, choose the correct date and the date will automatically enter into the box.
To insert hours, please choose the full hour and include the minutes in the next box in 15 minute increments. Round up if it is anywhere between the given minutes.
If you have more activities you would like to log, you may enter as many activities as you like up to 5 total. If you have more than 5, you may submit another form.
When you are finished, be sure to click "Submit". You will then be transferred to a receipt page. Please print the receipt for your records and file it away.
